Embolotherapy
Embolotherapy is a minimally invasive endovascular x-ray procedure. It consists in therapeutic introduction of various substances into the circulation to occlude vessels. Embolization, or embolotherapy, is performed by radiologists.
Embolization can be used as an independent method of treatment, and as a preoperative preparation. In this case, it is achieved to minimize blood loss and reduce the number of complications.
There are different types of embolization agents:
-
Coil embolization is a minimally invasive procedure to treat an aneurysm by filling it with material that closes off the sac and reduces the risk of bleeding;
-
stent graft implantation – is indicated in the presence of bleeding from the defect in the vessel wall;
-
microspheres - are used for the final closure of the arteries, for example, in uterine artery embolization in uterine myoma. Emboli microspheres are made of a special medical polymer and have a strictly defined size;
-
the particles of a polyvinyl alcohol - the material for permanent embolization;
-
ONYX glue - for the treatment of cerebral arteriovenous malformations.
Organizing narrowly focused courses with the foreign proctors involvement Daksmed Group of Companies trains Russian specialists and develops embolotherapeutic methods.
The full range of equipment for successful embolization procedure (microspheres, guide wires, microcatheters, introducers, etc.) is presented in the Terumo catalog.
Actual positions
Special catheters:
Terumo Сompany together with leading Russian experts have developed two types of non-standard catheters:
1. "Bobrov” catheter – for uterine artery embolization (UAE) through femoral access
2. "Babunashvili” catheter – for uterine artery embolization (UAE) through radial access
Taking into account the vast experience of Boris Bobrov and Avtandil Babunashvili in the endovascular technique of UAE, Terumo engineers accepted their proposals regarding the specifics of the Optitorque catheter tip. This made it possible to develop special types of Optitorque catheters, allowing to perform the procedures of UAE much faster and more convenient for patients. Both catheters have a steel double braided construction, except for the distal 2 cm; curved shoulder length is 19 cm; the bend angle of the distal tip - 90°; the presence of internal PTFE (Polytetrafluoroethylene) coating; polypropylene atraumatic soft tip catheters; the length of Babunashvili catheter – is 110 cm.
Microspheres:
Terumo's Department of interventional Oncology presents compressible microspheres for Hydro Pearl and LifePearl embolization. Microspheres Hydro Pearl is designed to occluzivee blood vessels in order to primary and adjuvant therapy. Terumo Interventional Oncology Division presents HydroPearl and LifePearl, a precise answer to a challenge in interventional oncology. HydroPearl are PEG (polyethylene glycol), biocompatible, compressible, non-drug-loadable and precisely calibrated embolisation microspheres. Available in a wide, color-coded range of sizes. LifePearl microspheres are used in the treatment of primary and secondary liver cancer and comprises a range of microspheres that are biocompatible, hydrophilic, precisely calibrated and capable of loading and releasing chemotherapeutic agents in a controlled manner.
Due to its composition, HydroPearl offers excellent properties and a very tight calibration. HydroPearl microspheres are intended to occlude blood vessels for therapeutic or adjunctive purposes in: hypervascularized tumors; hepatocellular carcinomas; uterine fibroids; benign prostatic hyperplasia; peripheral arteriovenous malformations; tumors of the neck, torso and skeletal system; bleeding, trauma and pre-operative reduction of bleeding.
According to the results of biocompatibility tests, HydroPearl and LifePearl microspheres do not have undesirable biological reactions. After embolization, the drugs do not cause an inflammatory reaction. Compressible microspheres allow quick delivery through a catheter, adapted to the size of the vessel and precise selective embolization.
The Microcatheters:
Progreat® microcatheters are ultra-selective hydrophilic radiopaque microcatheters with Unique varied tungsten coil pitch construction providing distal flexibility and proximal pushability, enhancing vessel selectivity and catheter placement. PROGREAT® Microcatheters are designed to allow navigation through tortuous peripheral vessels for optimal access and delivery of therapeutic embolization.
• Three-layer structure:
- PTFE inner layer is designed for exceptional guidewire tracking, with virtually frictionless delivery of coils and other embolics
- Middle layer - Radiopaque Tungsten spiral
- Outer layer: The varied materials (Polyester Elastomer) with M-coat of the outer layer provide stiffness for transition towards the distal tip of the microcatheter, ensu-ring higher trackability into narrow and tortuous target vessels.
• Unique varied tungsten coil pitch construction provides distal flexibility and proximal pushability, enhancing vessel selectivity and catheter placement.
• Reduced friction, selectivity and the highest level of navigation due to the hydrophilic coating, which prevents the adhesion of blood cells to the catheter surface.
• Catheter distal tip can be formed (the catheter is equipped with a mandrel to create a form).
• Radiopaque 0.7 mm platinum/iridium microcatheter markers engineered into 110, 130, and 150 cm microcatheters.
• High compatibility with a variety of embolic agents: coils, Liquid Embolic Systems, microspheres, as well as contrast media and chemo-embolic agents.
Coils:
The AZUR Peripheral HydroCoil Embolization System combines a platinum coil and an expandable hydrogel polymer. AZUR’s Hydrogel provides a barrier around the outer diameter of the core to prevent blood flow through the coil and minimize microchannel formation.
The technology of gradually expanding AZUR hydrogel provides:
- Patented Hydrogel technology swells at a defined rate in the presence of blood to provide additional filling and stabilization of the vascular space
- Provides a biologically inert scaffolding for natural tissue proliferation and shows lower rates of recanalization and repeat procedures
- AZUR® expanding Hydrogel and volume filling provides the potential to use fewer coils, which may provide cost saving per procedure
